Without shelter, the elements are harmful and potentially deadly \u2014 even here.<\/h2>\n<h6><em>By Joe Pye<\/em><\/h6>\n<p><span style=\"font-weight: 400;\">Mary Stewart was sleeping in a tent in the woods for two years by the time she first caught pneumonia.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">She coughed up phlegm and mucus non-stop into napkins and toilet paper from a local McDonald\u2019s. An entire week went by where she laid bundled in a coat, burning up from a fever while shivering from body chills, having no energy to eat or go out and panhandle.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">After seven days, her hunger grew too painful to rest any longer, so she pulled herself together and left the tent she called home in search of a can of soup from a nearby Winn Dixie. Up until then, Stewart says she survived solely on water and other liquids. The next day, she took an hour-long bus ride to the emergency room in JFK Medical Center in Lake Worth.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201c[The nurses] had to put me on oxygen and rush me in for a chest x-ray,\u201d says Stewart, who\u2019s also written for the Homeless Voice. \u201cThey said I had pneumonia on my left lung and admitted me for an entire week.\u201d\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">That was the middle of January in Palm Beach, Florida. Fourteen years \u2014 and one more bout of pneumonia \u2014 later, and Stewart still sleeps in a tent outside, chronically homeless since 2005. And her story is common to hear for homeless healthcare providers throughout the country.<\/span><\/p>\n<h3><em>Wintertime health risks for the homeless<\/em><\/h3>\n<p><span style=\"font-weight: 400;\">For those who are unsheltered \u2014 meaning living somewhere not fit for human habitation such as the street, an abandoned building etc. \u2014 the winter season can bring life-threatening health risks according to Dr. Courtney Pladsen.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Pladsen is the director of clinical and quality improvement at the <\/span><a href=\"https:\/\/nhchc.org\/who-we-are\/staff\/\"><span style=\"font-weight: 400;\">National Health Care Council for the Homeless<\/span><\/a><span style=\"font-weight: 400;\"> (NHCHC), a nonprofit organization that works with more than 300 clinics across the country training and sharing research, including in Portland, Maine where she is based.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The effects of cold weather on people experiencing homelessness are \u201call too familiar,\u201d to her, she says.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">One issue many patients come to her with is a condition called \u201ctrench foot,\u201d where they step in snow and can\u2019t dry off as they have nowhere warm and dry to go to. They can develop painful blisters that make it difficult to walk and can lead to infection.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Another risk is from the cold alone. \u201cI\u2019ve had patients who have had amputations because of frostbite,\u201d Pladsen says, \u201cand lots who\u2019ve had nerve damage.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">But the biggest health risk is hypothermia.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Hypothermia is when body temperature falls below 95 F, a \u201csevere medical emergency,\u201d according to <\/span><a href=\"https:\/\/nhchc.org\/wp-content\/uploads\/2019\/08\/Extreme_ColdV31.pdf\"><span style=\"font-weight: 400;\">the NHCHC<\/span><\/a><span style=\"font-weight: 400;\">. The heart, brain, and kidneys can malfunction, and lead to death.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cMalnutrition, decreased body fat, underlying infection, lack of fitness, fatigue, inadequate shelter and heat,\u201d are all risk factors for developing hypothermia as well, and all common in those experiencing unsheltered homelessness.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">According to the National Coalition for Homelessness\u2019 2010 report <\/span><a href=\"http:\/\/www.nationalhomeless.org\/publications\/winter_weather\/report.html#2\"><span style=\"font-weight: 400;\">\u201cBringing Our Neighbors in from the Cold,\u201d<\/span><\/a><span style=\"font-weight: 400;\"> relatively few people die directly from hypothermia, but rather complications that come from it. The report highlights how, \u201ceven those who seek shelter \u2013 and are allowed to enter \u2013 are frequently turned back onto the streets during the day,\u201d leaving them to bear the cold once again.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201c700 people experiencing or at risk of homelessness are killed from hypothermia annually in the United States,\u201d the same report estimates. This is because the risk of hypothermia is not solely tied to outdoor ambient temperature, but also wind-chill, amount of clothing to protect, and whether that clothing is even dry.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Many people experiencing homelessness are also experiencing alcohol and substance abuse dependency. Alcohol, nicotine, street drugs, and some medication all increase the risk of developing hypothermia, according to <\/span><a href=\"https:\/\/nhchc.org\/wp-content\/uploads\/2019\/08\/Extreme_ColdV31.pdf\"><span style=\"font-weight: 400;\">the NHCHC<\/span><\/a><span style=\"font-weight: 400;\">.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">This is something Stewart has experienced and was surprised about. Throughout the 16 years she\u2019s been on and off the streets, friends of hers have died due to alcohol and drug dependency.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cI didn\u2019t even know that,\u201d she says. \u201cI\u2019ve known a lot of people who would drink whiskey when it\u2019s 45 degrees out. Imagine people in New York or New Jersey.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In Pladsen\u2019s experience, northern cities are rarely prepared to handle the amount of people who need shelter in the winter months. Most cities nationwide, she says, lack cold-weather response plans. To make matters worse, the COVID-19 pandemic has lessened volunteers, closed shelters, and lowered the amount of beds available, generally amplifying challenges for homeless service providers and the communities they assist.<\/span><\/p>\n<h3><em>An \u201cunprecedented winter\u201d<\/em><\/h3>\n<p><span style=\"font-weight: 400;\">Before the pandemic, emergency shelter beds for cold weather were <\/span><a href=\"https:\/\/beta.homelessvoice.org\/cold-and-covid-how-the-winter-could-outrank-the-virus-in-deaths-of-the-homeless\/\"><span style=\"font-weight: 400;\">already too few<\/span><\/a><span style=\"font-weight: 400;\"> for the amount of individuals experiencing unsheltered homelessness \u2014 192,793 too few. According to that same 2010 report, out of a sample of 50 <\/span><span style=\"font-weight: 400;\">emergency winter shelters <\/span><span style=\"font-weight: 400;\">across the country, only 20 opened above 32 F, the rest opened either when it dropped even farther, or to the shelter\u2019s own discretion at the time.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">This leads to the possibilities of these health risks Pladsen was weary of, as those needing the shelter are forced back outside during the day again \u2014 if they had found a bed to begin with.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Last year, cities had specific funding called <\/span><a href=\"https:\/\/www.hudexchange.info\/programs\/esg\/esg-cv\/#program-requirements\"><span style=\"font-weight: 400;\">ESG-CV funding<\/span><\/a><span style=\"font-weight: 400;\">, an Emergency Solutions Grant through the CARES Act. Cities could use ESG-CV funding for hotels and motels to decompress shelters and house people during these cold months.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">But this year, many cities and municipalities have run out of that money.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Shelters that were overburdened prior to the pandemic have been cut to half capacity, making it that much tougher to find beds to sleep during winter months. Many soup kitchens throughout the country have closed as well due to the pandemic and there are fewer volunteers to help, according to Pladsen.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The United States Department of Housing and Urban Development (HUD) tracks the number of \u201csheltered and unsheltered people experiencing homelessness on a single night,\u201d called a \u201c<\/span><a href=\"https:\/\/www.hudexchange.info\/programs\/hdx\/pit-hic\/\"><span style=\"font-weight: 400;\">Point-in-Time Count<\/span><\/a><span style=\"font-weight: 400;\">.\u201d That single night is in January and the count is repeated annually.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The last count was in January 2020, just before the pandemic began, and Pladsen points out, the first time since the count began in 2007 <\/span><a href=\"https:\/\/beta.homelessvoice.org\/homelessness-pre-pandemic-increases-for-fourth-consecutive-year\/\"><span style=\"font-weight: 400;\">that unsheltered individuals outnumbered sheltered ones.<\/span><\/a><span style=\"font-weight: 400;\"> \u201cSo more people are opting to sleep outdoors than in shelters,\u201d she says. \u201cAnd that was before the pandemic.&#8221;<\/span><\/p>\n<p><span style=\"font-weight: 400;\">All signs suggest that this season will be more severe with what Pladsen calls, \u201ca winter with unprecedented challenges.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cPrograms that were online last year for winter in many places are now closed with nothing else new coming online,\u201d she says. \u201cThey\u2019ve closed hotels and motels and that funding is over. Shelters are not opening more beds and there are no new ones being built. So it has created a particular challenge this year from last year.\u201d<\/span><\/p>\n<h3><em>Florida and the unhoused: little shelter, little health care<\/em><\/h3>\n<p><span style=\"font-weight: 400;\">Florida is a non-Medicaid expansion state under the Affordable Care Act, and that makes for a disproportionate rate of uninsured residents in the state, says Pladsen. This is because, according to the <\/span><a href=\"https:\/\/www.benefits.gov\/benefit\/1625\"><span style=\"font-weight: 400;\">Florida Medicaid Benefits site<\/span><\/a><span style=\"font-weight: 400;\">, only those who are pregnant, responsible for a child, living with a disability or have family members with a disability, or are 65 years of age and older qualify. Without some form of health insurance, there\u2019s a barrier for physical and mental health care, she says.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Stewart, being diagnosed with high-functioning autism and bipolar disorder, is eligible for Medicaid in the state of Florida, but not everyone going through the homeless experience has similar resources.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Pladsen recalls a patient who fled the cold in Portland only to find fewer resources in South Florida.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cHe heard it might be easier to get away from the bad weather,\u201d she says. \u201cHe found out he couldn\u2019t find any shelter or health care the entire six months he was there. So he just ended up coming back to Maine. We hear that people can\u2019t find services and get out of homelessness a lot in Florida.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Eligibility for services in Florida doesn\u2019t necessarily mean being taken seriously for those issues though.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Stewart\u2019s hour-long bus ride to Lake Worth wasn\u2019t her first attempt to treat her pneumonia. She had visited a hospital run by her caseworker, located within walking distance of her camp. The caseworker didn\u2019t believe she was sick and wanted to admit her into a psychiatric ward instead, Stewart says.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cI was curled up in a ball crying in the waiting room,\u201d Stewart says. \u201cThe doctor told me he\u2019s been sick before and never acted like that.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In response, she hopped on the Palm Tran, the local public transportation bus system. A hospital located only 11 miles and 17 minutes away by car took one hour and two buses to get to.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Stewart has also experienced homelessness in Kentucky and North Carolina. She\u2019s stayed in homeless shelters everywhere she\u2019s experienced homelessness, except for Florida. The nearby shelters to her like the Lewis Center aren\u2019t welcoming in her experience.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">In Louisville, Kentucky, she says you can\u2019t walk more than three blocks without finding another place offering meals to the homeless.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">She theorizes Florida\u2019s lack of services has a lot to do with the tourism driven economy in the state.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cThey have all of these tourists coming down and don\u2019t want eyesores in the community,\u201d Stewart says. \u201cSo they refuse to help the homeless to try and deter them from being here because they don\u2019t want their tourism dollars affected. In other states there actually is more help for the homeless.\u201d\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Others living through the homeless experience in Florida have similar criticisms and tell the Homeless Voice the same. One Broward County man said the last time he stayed at a shelter it was infested with bed bugs. So now he opts to sleep in his car.<\/span><\/p>\n<h3><em>How the unhoused can prepare for the winter<\/em><\/h3>\n<p><span style=\"font-weight: 400;\">Stewart has learned more than a few lessons since the last time she was diagnosed with pneumonia. She\u2019s since been vaccinated for pneumonia, COVID-19 and the flu.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">As someone who\u2019s experienced homelessness on and off for 16 years, she advises to never keep wet clothing or blankets, but acknowledges that\u2019s tough to do in a rainy climate.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Palm Beach State College offers a homeless exemption program, in which Stewart has enrolled and already graduated with an associates degree. That gives her the ability to also qualify for low-income student resources on campus like <\/span><a href=\"https:\/\/www.palmbeachstate.edu\/panthers-closet\/default.aspx\"><span style=\"font-weight: 400;\">Panther\u2019s closet<\/span><\/a><span style=\"font-weight: 400;\">, a thrift store where all items are only one dollar for students. She\u2019s able to purchase winter clothing like sweaters, long pants and long sleeve shirts there.\u00a0\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Years ago, Stewart was assigned a lawyer because she had to give her children up for adoption. She and the lawyer stay in touch and on occasion she receives meals, blankets and other gifts from her.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">But these problems are bigger than a few blankets and dry clothes.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Pladsen recommends cities look into winter planning resources from organizations like HUD. To her, the major issue noticed by the unhoused sources in this story is how shelters attempt to operate on their own. There needs to be a healthcare provider partnered with the shelters, she says.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">States, cities and municipalities do have what\u2019s called <\/span><a href=\"https:\/\/www.hudexchange.info\/programs\/coc\/\"><span style=\"font-weight: 400;\">Continuums of Care<\/span><\/a><span style=\"font-weight: 400;\">: a program that helps fund shelter systems and federally-supported housing, among other efforts like organizing the point-in-time count.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Pladsen uses <\/span><a href=\"https:\/\/sulzbacherjax.org\/about\/\"><span style=\"font-weight: 400;\">Sulzbacher<\/span><\/a><span style=\"font-weight: 400;\"> in Jacksonville, Florida as an example for what shelters in South Florida should model themselves after. Sulzbacher was founded in 1995 with the mission that, \u201chomeless persons need and deserve more than just a meal and a bed to end their homelessness.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Sulzbacher took the Continuum of Care approach to use funding for three entities: housing, income, and healthcare.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Pladsen says to better serve communities, Continuum of Cares must bring together the intersection of health and housing to help people get out of the cycle of homelessness.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u201cWe\u2019re not going to address this complex challenge with one single solution,\u201d Pladsen says. \u201cWe really need all hands on deck to acknowledge this isn\u2019t working.\u201d<\/span><\/p>\n","protected":false},"excerpt":{"rendered":"<p>Temperatures in South Florida can drop below 40 degrees.
